Provider Acknowledgments

Provider acknowledges, understands, and agrees to proceed with the following requirements:

  1. Mutual NDA execution.
  2. Review and acceptance of the Master Services Agreement (MSA) Terms and Conditions.
  3. Review and acceptance of Program Technical Integration, Reporting, and Performance Requirements.
  4. Completion and acceptance of Statement of Work (SOW) requirements.
  5. Completion of all required technical integrations.
  6. Integration with the Billing Portal.
  7. Production launch (Go Live).
